HIV Case Management Services
This program provides individualized care through assignment of a case manager to address the range of needs addressing each client diagnosed with HIV/AIDS. The case manager will meet with the individual and develop a care plan, which identifies all of the needs to be addressed and a plan for addressing them. Needs range from physical to psychosocial to concrete needs. The case manager will assist the client in prioritizing needs, then locating the resources to meet those needs. The case manager will then set up regular meetings with the client, with the frequency and location determined by the client and case manager. The case manager can make home or hospital visits, as needed. A limited amount of HIV Emergency Funds are available through this program for enrolled clients with specific emergent needs (e.g., utility shut-off, lack of food, lack of medication, etc.).
