How to Apply for Services
Counselors will take brief information over the phone and, if you wish, schedule an appointment, usually within a few days. All services provided by Somerset Treatment Services are totally confidential, from the very first phone call. No information can be released without your signed consent except where state law requires disclosure. We ask you to bring in proof of income when you come for your first visit (Pay stub, Medicaid card, income tax forms, insurance information, disability or other entitlement documentation, etc.). This will help us determine your sliding scale status, Medicaid eligibility, etc. Email form  |
